Ghana visa application form. Completed application form for Ghana will be prepared within 5 business hours of order submission, and emailed to you to download, print and sign. Passport copy.A clear scan of the information page of your valid, signed passport.

Passport-type photographThe photo must be taken within the last 6 months, it should be in color against a flat white background, without any shadows and light spots. All facial features in the photo must be clearly visible, the presence of foreign objects is not allowed.

If neither has a valid Ghanaian passport, then a copy of the passport they hold and Ghanaian permanent resident card is acceptable.

Health Declaration Form. Passengers must also have a completed Health Declaration Form before entering to the country.

• an undertaking that the host will ensure that the visitor complies fully with immigration laws of Ghana/ Invitation Letter can be used

• copies of Bio data pages of passport

• resident Permit of all non Canadians

• evidence of Travel Itinerary

• birth certificate of child under 18 years
